Job Title: Home Care Nurse SpecialistJob Summary:Talent Shark is seeking a skilled Home Care Nurse Specialist to provide high-quality nursing care to patients in their homes. The ideal candidate will be responsible for conducting comprehensive assessments, developing individualized care plans, and providing a wide range of nursing services.Key Responsibilities:Patient Assessment: Conduct thorough assessments of patients in their homes, including evaluating their medical condition, environment, and care needs.Care Planning: Develop tailored care plans in collaboration with physicians, therapists, and other healthcare professionals.Nursing Care: Provide wound care, medication administration, monitoring vital signs, managing chronic conditions, and assisting with activities of daily living (ADLs).Patient Education: Educate patients and their families on managing their health, including medication management, dietary guidelines, and proper use of medical equipment.Health Monitoring: Regularly monitor patients’ health status and update care plans as needed.Coordination of Care: Coordinate with other healthcare professionals to ensure seamless care, including arranging follow-up appointments, lab tests, and specialist consultations.Documentation: Maintain accurate and up-to-date patient records, including care plans, treatment notes, and any changes in the patient’s condition.Patient Advocacy: Act as an advocate for patients, ensuring they receive the necessary care and support while respecting their autonomy and preferences.Compliance: Adhere to all hospital policies, protocols, and regulatory standards, including those related to patient confidentiality, infection control, and safety.Qualifications:Education: Bachelor’s degree in Nursing from an accredited institution.Licensure: Must hold a valid nursing license recognized by the Dubai Health Authority (DHA).Experience: Minimum of 2 years of nursing experience with at least 1 year in a home care or community health setting.Skills:Strong clinical skills and the ability to provide high-quality nursing care in a home setting.Excellent communication and interpersonal skills with the ability to build rapport with patients and families.Ability to work independently and make informed decisions in the field.Cultural sensitivity and the ability to work in a multicultural environment.Languages: Proficiency in English is required; knowledge of Arabic is an advantage.Driver’s License: A valid UAE driver’s license is required as the role involves traveling to patients’ homes.Working Conditions:Hours: Full-time position with flexible working hours depending on patient needs.
